Cottonwood students who would like to work while attending school will need to obtain a work permit. There are two types of work permits for minors: Non-Entertainment and Entertainment

 

Non-Entertainment Work Permits

To obtain a non-entertainment work permit for a minor, families can download a Statement of Intent to Employ a Minor and Request for Work Permit (form CDE B1-1) from the CA Department of Industrial Relations website or get the form from the potential employer.

After filling out the top portion of the form, the form should be emailed to Karson.Davis@Cottonwoodk12.org with the words “Work Permit” and the student’s last name in the subject line (i.e., Subject: Work Permit – Smith).

The Records Department will receive this form and process it within 4-5 business days. They will verify the student is active and currently working in compliance with Cottonwood’s policies. After verification, the Records Department will fill out and sign the bottom portion of form CDE B1-1 and complete form CDE B1-4, which is the actual permit. Both forms will then be returned to the family.

Entertainment Work Permits

To obtain an entertainment work permit for a minor, families can download the Application for Permission to Work in Entertainment Industry (form DLSE 277) from the CA Department of Industrial Relations website: https://www.dir.ca.gov/dlse/Entertainment-Work-Permit.htm

After filling out the top portion of the form, the form should be emailed to Karson.Davis@Cottonwoodk12.org with the words “Work Permit” and the student’s last name in the subject line (i.e., Subject: Work Permit – Smith).

The Records Department will receive this form and process it within 4-5 business days. They will verify the student is active and currently working in compliance with Cottonwood’s policies. After verification, the Records Department will sign, stamp, and provide the family with two copies. One will be sent via email, the other through regular US mail.

The family will then need to send the completed application back to the CA Department of Industrial Relations. This may be done online, by mail, or in-person. For details on where to return the application, please visit the DIR website.
